Imaging techniques for diagnosis and management of ankle instability

The fourth day of the Sports Med Days is focused on “Ankle Instability,” a common and significant issue in elite sports frequently resulting from acute or chronic ligament injuries1. For this reason, accurate diagnosis is essential to prevent long-term complications and to guide rehabilitation strategies that support a safe return to sports activities.

Medical imaging (ultrasound and MRI) plays a crucial role in assessing ankle instability: it is essential for identifying injuries that may be overlooked and tailoring treatment plans to the athlete’s needs.

Join the Sports Med Days series on Day 4 for a focused session on medical imaging in ankle instability, and learn how ultrasound and MRI complement each other in diagnosis, treatment planning, and rehabilitation for athletes with ankle injuries.

Presentation: Ultrasound Evaluation of Ankle Instability in Athletes

Professional athletes often present with ankle sprains that account for 16-40% of sports-related injuries, and up to 85% of those sprains affect the lateral complex.
Although most of the cases respond appropriately to conservative management, about 20% to 30% can develop chronic ankle pain and instability
Appropriate diagnosis is key to developing a comprehensive treatment plan, and imaging studies play a fundamental role in complementing the patient's clinical picture.

As X-ray may be considered the first line of assessment, looking for bone abnormalities related to an ankle sprain, a complementary assessment may be done with an ultrasound scan, checking on ligaments compromised and stability when associated with dynamic manoeuvres. This leaves MRI to rule out intraarticular injuries and/or complex ligament injuries.

Presentation: MRI Assessment of Ankle Instability in Sports Injuries

Ankle instability in sports has become much more important as treatment approaches have changed, increasing the number of athletes who undergo surgery because of chronic pain or instability. Frequently, a combination of acute and old injuries is seen together in these sports athletes.

To accurately report ankle ligament injuries, the knowledge of the anatomy, biomechanics, and new special injuries, such as retinaculum injuries, is essential, without forgetting the complications, osteochondral injuries, and secondary impingements. For educational purposes, they are divided into high sprain, lateral, and medial injuries, although not infrequently they occur together.

The session will review the role of the radiologist in ankle sprains in acute and chronic settings.
